To get an outgoing trunk to not show up as "unknown" you have to include the CID if your SIP provider isn't suppling it themselves. (Typical telco's typically seem to provide it for you, too.)
The proper syntax is:
"Mindwatering" <5405551212>
Include the quotes and the brackets, they are not place holders.
